Practice Focus

Dr. Roache is a fellowship trained, Board Certified Orthopaedic surgeon. Whose main practice is in San Francisco. He is also an attending surgeon during ski season at Mt. Crested Butte Ski Clinic in Colorado.
He is an expert in the treatment of Shoulder and Sports injuries.
His main Subspecialty focus is in Arthroscopic treatments of Rotator Cuff disorders, Frozen shoulder and Shoulder dislocations.
In addition, he has strong specialty interest in the Arthroscopic treatment of Sports Medicine injuries to the Knee.
Dr. Roache has extensive training and experience in trauma and fracture treatments. And he continues to treat non-emergent fractures and trauma.
Recently, he has developed a working interest in Yoga to treat orthopaedic injuries. He is currently working with local yoga teachers and their students to develop techniques for injury treatment and prevention.
Dr. Roache actively collects data for research and outcome studies. And actively participates in National and International Orthopaedic Conferences.

Educational Background

Dr. Paul Roache (pronounced “rowch”) began his studies at The University of California at Berkeley, where he was on their Division 1 track and field team as a pole-vaulter.
From there, his studies took him to Loyola Marymount University in Los Angeles Where he received his degree in Biology. He took a break from his studies to train and compete at a national level in the pole-vault as an Olympic hopeful. Resuming his studies he earned his Medical degree from the University of Arizona, College of Medicine in Tucson Arizona.  After internship, he completed his first year of orthopaedics as a fellow on the trauma service of Boston City Hospital. Dr. Roache moved to New York City to complete his Orthopaedic training at Albert Einstein in the Bronx, New York. There he was a Chief Resident on the Trauma service at infamous Lincoln Medical Center, a level one-trauma center in the South Bronx.

Seeking a more in-depth study into his subspecialty, Dr. Roache completed a one year fellowship in Shoulder and Elbow reconstruction, under the direction of internationally acclaimed and a past president of the American Shoulder and Elbow Surgeons, Tom R.Norris, MD, and his partner Relton McCarroll, MD, a nationally acclaimed Elbow and Hand surgeon.

Next his studies took him to The University of Balgrist in Zurich Switzerland as an AO Fellow. There he studied the advanced Rotator Cuff repair techniques with world renowned Professor Christian Gerber, MD. In 2004, He was invited to visit Professor Jon J.P. Warner, Director of Shoulder Service at the Massachusetts General Hospital in Boston to study his arthroscopic techniques.

Professional Background

Dr. Paul Roache has a mastery of advanced Shoulder arthroscopic surgical techniques. He sub specializes in treating Rotator Cuff disorders, Frozen shoulder and instability disorders of the shoulder. By using arthroscopic techniques, one can perform even complicated shoulder surgeries through minimally invasive incisions, resulting in less pain and easier rehabilitation for patients.  Dr. Roache has performed over 500 arthroscopic surgeries in nearly 10 years of practice as a Shoulder and Sports medicine specialist.

In addition to his Shoulder and Sports Medicine practice in San Francisco. Dr. Roache is an attending orthopaedic surgeon at he Mt. Crested Butte Ski Clinic, in Colorado, where he treats trauma and sports medicine injuries from snow sports and works with Ski patrol on injury prevention.

He is a candidate member of the Arthroscopic Association of North America. A fellow in the American Academy of Orthopaedic Surgeons. He is Board Certified by the American Board of Orthopaedic Surgeons. He is a candidate for the Orthopaedic Board Sports Medicine Certificate.

Personal Background

An avid Swimmer, he has completed numerous swims from Alcatraz, including the Escape from Alcatraz triathalon. An accomplished skier, He enjoys scuba diving, Salsa dancing, and cycling as well. A former aerobatic pilot and Skydiver, he is a licensed pilot. He has a strong hatha yoga practice and has recently been working with yoga teachers to identify shoulder dysfunction in their student’s practices.
A native of San Francisco, a graduate of St. Ignatius College preparatory. He lives in San Francisco.
